DailyVitals is a straightfoward application that helps GPs and other clinicians get more frequent, reliable data to help them diagnose the wellbeing of patients in care homes via a realtime dashboard without having to visit the care home patients every day.
This allows GPs and other clinicians to make expert opinions earlier and more rapidly. Earlier interventions can help to reduce the likelihood of a health condition getting out of hand. And therefore reduce the need for potential need for a hospital visit.
One major goal is to shorten the time from observing a worsening condition to contacting the GP. A click to call GP button, allows care home staff to contact the GP conveniently and immediately, should the need arise.
